Incidents - General Hospital - WHO taxonomy


This is the renewed version of the Incident tracking for a General Hospital following the WHO taxonomy(*).

The tracking may proceed in 4 phases via a browser (each staff member has one or other browser on his/her PC for that matter). In your hospital you will put the relevant icon(s) on the staff member's PC.

1. The Reporter registers a limited number of elements (each hospital can choose which ones). This is called Level-1. It may also select the types of incidents it wants to register separately and e.g. combine the other types under 'Other'. Each registration is given a unique number. At the end of his/her registration, the Reporter can send the registered form by email to a number of competent persons (typically different persons for each different type of incident). He/she may INTERRUPT the registration at any time and come back later yo complement it (Level-0).

2. The Reporter calls the 'old' incident and completes it. This is called Level-0. At the end, he/she will answer the question whether the registration is complete yes/no.

3. The Competent person(s) can download this registered form (by entering its unique number) and can then correct/complement the form. Also these managers/responsible staff members can send an email (to other competent persons) that contains this (modified) form, asking them to inquire and/or complement the form further. He/she is also confronted with the question 'Is this incident IMPORTANT?'. If yes, he/she will register the ECM-codes and PRISMA improvement-actions. LOGIN/PASSWORD = alfa/alfa, beta/beta, gamma/gamma. Also here, the registration may be interrupted. This is Level-2.

4. Finally, the Incident Manager will then decide when to close all modifications: he/she again loads the form (by entering its unique number) and can still correct/complement the form. He/she will also supply the retroactive analysis by entering the ECM-codes for the incident. Some data are rendered anonymous. At the end of this final registration, the temporary form will disappear and the final registration will occur into Snap. Possible modifications can thus still occur later in Snap. With this Level-3 the Incident manager decides to make the final registration. LOGIN/PASSWORD = delta/delta.

Snap will perform very comprehensive statistical analyses, with or without using the WHO-taxonomy.
Both the generated emails and different data structures exported out of Snap may then be embedded in any kind of document handling system, CSM, ERP,...
